Postpartum psychosis: an emergency, not a positivity problem

Postpartum psychosis can begin abruptly, often in the first two weeks, and requires emergency assessment. It is not synonymous with ordinary depression or an inevitable consequence of it.

The important distinction

A wellness program cannot predict or prevent every rare crisis. Specialist planning is important with bipolar disorder, prior postpartum psychosis or relevant history. Treatment can involve hospital care and specialist interventions.

Psychosis needs urgent professional care

Population and setting: Postpartum psychosis, approximately 1–2 per 1,000 births.
What was found: Usually begins abruptly in the first two weeks; hallucinations, delusions, mania or severe confusion warrant emergency assessment.
Limits and applicability: It is distinct from ordinary PPD and from unwanted intrusive thoughts. No app can guarantee prediction or prevent all tragedies.

ECT remains a specialist option for severe illness

Population and setting: Severe postpartum psychosis or other life-threatening/refractory presentations.
What was found: Guidance includes ECT in selected severe cases; 2026 review summarizes a limited predominantly nonrandomized literature.
Limits and applicability: Not an outpatient self-help intervention. Urgency and specialist judgment matter more than app engagement.

What should a family do now when signs appear?

Arrange a trusted adult to remain with and care for mother and baby. In the US, call 911 or go to an emergency department. Do not wait for an app or email response.

Use this as information, not an assessment

Symptoms or concerns deserve appropriate professional care. Severe or worsening symptoms should not wait for an app, a quiz or a two-week threshold.
